Answer: Since you’re going from a stereo source to a mono amp, you’ll have to do it in two steps.
First, you’ll need a 3.5mm male to two 1/4" males:

Then you’ll need to combine the channels and go into your amp. You’ll need a "Y" cable that goes from two 1/4" females to one 1/4" male:
